Miracle Home Makeover Terms & Conditions

This Miracle Home Makeover raffle ticket is offered by the Samaritan Foundation, a New York State Not-for-profit corporation under Sec. 510 (c) (3) of the Internal Revenue Code to benefit Children’s Miracle Network of Northern New York (CMN). Ticket price is $50. Payment for your ticket may be made by cash or check payable to Children’s Miracle Network or funds credited to CMN against a donor’s MasterCard, VISA or Discover charge account. If your entry ticket is received by Wednesday, November 17, 2010 prior to 4 p.m., the Samaritan Foundation will enter the stub of this ticket into the Miracle Home Makeover Raffle. The drawing for the Miracle Home Makeover will be held on Wednesday, November 17, 2010 at 6 p.m. at the new Samaritan Medical Center Foster Atrium. Donor need not be present to win. The winner of the raffle will be selected at random from eligible entries and will be notified by certified mail to the address given on the stub within 10 business days after the drawing. The winner must present this ticket and identification to claim the prize and has until 5 p.m. on the fifth business day after the postmark of the mailed notice to do so. If the prize is not so claimed by the deadline, the next drawing ticket may be awarded the prize, subject to presentation of that ticket and identification by 5 p.m. on the fifth business day after the postmark of the mailed notice to such party. Copies of these terms may be obtained by mailing a stamped, self-addressed, return envelope along with a request for such a copy or copies to Children’s Miracle Network, 830 Washington Street, Watertown, NY 13601. Requests to purchase tickets by mail for the Miracle Home Makeover must be postmarked no later than Wednesday, November 10, 2010, and received prior to the drawing. The maximum number of tickets issued will be 4,500 and requests for tickets will be processed in the order in which they are received. After November 17, 2010, to receive the number of the winning ticket please send a self-addressed, stamped envelope to CMN, 830 Washington Street, Watertown, NY 13601 with your request. By entering the Miracle Home Makeover Raffle, the winner agrees to the Makeover project valued at $100,000 in products and services from the Miracle Home Makeover participants net of taxes or the cash equivalent net of taxes. The Makeover project specifications, timeframe, location and scope of the work are to be approved by Children’s Miracle Network and ConTech/Premier Homes Building Systems. The Miracle Home Makeover will be constructed with funds supplied by Children’s Miracle Network from proceeds of this drawing. The remaining proceeds will benefit Children’s Miracle Network of Northern New York at Samaritan Medical Center. The number of entries received will determine the odds of winning.
